If your surname has changed subsequent to marriage and you want a new passport or an additional booklet in your married name you will need to fill out the form meant for a new passport and an affidavit of change of name. You also need to enclose your current passport, your original marriage certificate with a photocopy and your spouse's passport. At present, the fee for change of name subsequent to marriage is Euro 12/-.
