Application for change of name requires issue of a new passport. Applicants for change of name due to marriage, re-marriage or divorce are required to submit a certified marriage certificate and a joint affidavit in the prescribed format for change of name from husband and wife along with the joined photograph/ divorce deed or death certificate of spouse dully authenticated/ divorce deed authenticated by a court, respectively. In other cases, two original newspaper clippings from newspapers with wide circulation at their normal place of residence in India and an affidavit in the prescribed format for change of name.
